If you’re not immersed in it daily, it’s hard to grasp the sheer volume of steps involved pre consult for a new patient.

Let’s instead imagine if buying a coffee were like getting specialist consultant care:

Before making you a latte, the barista needs a referral from your local "Caffeine Manager," along with a stack of paperwork detailing your caffeine consumption habits, energy needs, and past coffee orders. The barista's assistant uploads this into the "Electronic Caffeine Record" and prints a copy for the barista. If anything is missing, the referral is sent back to the "Caffeine Manager" to fill in the blanks, and delaying your coffee.

Once all the necessary data is there, the barista reviews and assigns you a coffee queue priority and has their assistant schedule an appointment for your latte in the “Customer Management System”. They tell you before attending, you’re required to complete an energy test, fill out a detailed caffeine history form, abstain from coffee for 12 hours, and arrange a lift home. On the day of your appointment, you must bring your Keep Cup and a comprehensive list of all coffees consumed that week. You’ll receive multiple reminders about your appointment but no help preparing for it. After weeks of waiting, you might—just might—get your latte. Oh, and the barista then writes a detailed letter to the "Caffeine Manager" about the latte’s composition and delivery.

While light-hearted, this illustrates how pre-visit admin functions today—necessary but burdensome, diverting clinicians and team from patient-facing work during a time of critical staffing shortages and creating frustration for all involved.

    Xiao studied Computer Science and Computational Linguistics, earning a Master’s in Speech & Language Processing from Trinity College Dublin. Prior to Alto, he worked for Meta’s Oculus and Longcheer, honing his expertise in AI and engineering.

    Michael earned his PhD and completed his medical studies at Trinity College Dublin. He is a partner in a group practice, HSE Clinical Lead and Consultant in Anaesthesia and Intensive Care Medicine.
